Job Description:
A company is looking for a Care Coordinator - Clinical
Operations to provide outreach services and support to clients.
Key Responsibilities
Conduct Social Determinants of Health (SDOH) screening assessments
via phone and other channels
Make referrals to the in-house clinical social work team based on
assessment results
Document all inquiries and maintain professionalism during customer
interactions
Required Qualifications
High School Diploma or equivalent
1+ years of experience in accessing community resources
Knowledge of managed care preferred
Bi-lingual Spanish and English language proficiency required
Hardwired internet connection with specified speed requirements
