Description The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care. • Bachelor's Degree
• Upon hire: California Registered Nurse License
• 2 years of Acute hospital experience, including one year of supervisory experience. Preferred qualifications:
